In addition, please report known or suspected abuse/neglect/exploitation of a child or vulnerable adult to the state's Abuse Hotline at 1-800-962-2873 or online at https://reportabuse.myflfamilies.com/s/ or call 911.
Otherwise, the Office of Citizen Services will address online complaints and messages as quickly as possible.

If you choose to provide information in a feedback form, conference registration form or other form on this site, unless specifically exempted by Florida Statutes, such information is a public record under Florida's Government in the Sunshine Laws and available for public inspection upon demand.

ADA NOTICES AND PROCEDURES:
The Americans with Disabilities Act of 1990 (“ADA”) sets forth certain protections against discrimination for those with disabilities. Federal regulations that address these requirements as applied to services offered by state and local governments are codified here: ADA Federal Regulations.
Consistent with the ADA, the following grievance procedure applies to anyone who wishes to file a complaint alleging discrimination on the basis of disability in the provision of services, activities, programs, or benefits by the Florida Department of Legal Affairs (“Department”).

Complaints may be submitted via mail, email, or fax. Alternative means of filing a complaint for persons with disabilities will be made available upon request. All complaints and other communications sent to the Department are public records under Florida law and may be subject to disclosure upon request.
In order to assist us in evaluating your complaint, please provide:
- The name, address, and phone number of the complainant.
- The name of the victim or other witness of the alleged discrimination, if it is someone other than the complainant, if known.
- The basis for the complaint. Please include as much detail as possible.
- The date of the discriminatory conduct.
- The name and title of the person(s) who is alleged to have engaged in the discriminatory conduct.
The ADA Complaint Coordinator will provide written notice to the complainant of the receipt of the complaint and will explain to the complainant efforts that have been made or will be made to resolve the complaint. Upon request, the Department may communicate with the complainant in an alternative format. The Department will make appropriate arrangements to ensure that disabled persons are provided other accommodations, if needed, to participate in this grievance process. The ADA Complaint Coordinator will be responsible for such arrangements.
